Vacation Leave

Full-time Associates who have completed three months of continuous employment. (minimum of 32 hrs/wk)

Two weeks - Zero months through 60 months of service (3.077 hour per pay period)
Three weeks - 60 months through 120 months of service (4.615 hours per pay period)
Four weeks - 120 plus months of service (6.154 hours per pay period)

Sick Leave

Full-time Associates who have completed three months of continuous employment (minimum of 32 hrs/wk).

Accumulate one 8-hour day of sick leave per month. Associate may accumulate up to 130 days (1040 hours). Sick leave will not be paid in less than one (1) hour.

Holiday Leave

Full-time Associates who have completed one month of continuous employment (minimum of 32 hrs/wk).

Bereavement

Full-time Associates who have completed three months of continuous employment (minimum of 32 hrs/wk).

Tuition Reimbursement

Applications for tuition reimbursements must be submitted to the Human Resources Department prior to registration for course.

Available to full-time Associates with one (1) year or more of service; 100% paid with a maximum of $2,000 each calendar year. Must receive a grade of B or better. Must sign 1 year agreement.

Life Insurance

Full-time Associates (minimum of 32 hrs/wk) must complete one month of continuous employment.

Basic Plan - One time base salary (non-exempt Associates)
                       Two times base salary (exempt Associates)

Optional life also available.
Spouse/Dependent life available.

Long Term Disability

Full-time Associates (minimum 32 hrs/wk). New Associates will be subject to a one month waiting period before coverage becomes effective.

Provided at no cost to Associate.

Short Term Disability

Full-time Associate (minimum 32 hrs/wk). New Associates will be subject to a one month waiting period before coverage becomes effective.

401(k) Retirement

Plan provided in the form of a 401 (k) savings plan. Third party administrator: PRUDENTIAL. Full-time / Part-time Associates who work at least 20 or more hours week / 1000 hours each year are automatically enrolled with a 4% salary reduction upon employment. Vesting begins on your date of hire. Six year graduated vesting schedule.

Medical Insurance

Full-time Associates (minimum of 32 hrs/wk). Two plans are available; the plans are the same except for deductibles, co-insurances and out-of-pocket maximums.

Services in HMA Hospital - 100%
1. Inpatient and selected outpatient procedures require pre-certification.

Must use your HMA Hospital if service is available.

Must use Affordable PPO Hospital for services not available here or co-pay reduced from 80% / 20% to 60% / 40%.

Prescriptions

Prescriptions are covered through a prescription drug program administered by CVS/Caremark. The annual deductible is $50 with a $100 family deductible. Once the deductible is met, you pay a prescription co-payment of $10, $30, or $45 retail, depending on the prescription. Mail order co-pays $20, $60, $90 for a 90 day supply.

Unum / Allstate

Full-time Associates (32hrs/wk). A flexible spending account feature, which allows Associates to pay for unreimbursed medical expenses as well as childcare expenses on a pre-tax basis thru Flexible Corporate Plans.

Full-time and part-time Associates may purchase critical illness and cancer insurance through Allstate and may also purchase accident insurance and whole life insurance through the Unum Insurers. Premiums are paid on a pre-tax basis.

Dental Insurance

Full-time Associates (minimum of 32 hrs/wk). Must complete one month of continuous employment.

    $50 deductible per individual or $150 per family

Dental Premiums:

Associate Only $12.50 / pay period
Associate / Children $21.25 / pay period
Family $30.25 / pay period

Vision

Full-time or Part-time Associates.

One eye exam every twelve months, $10 co-pay, single vision bifocal and trifocal lenses every twelve (12) months. Retail allowance for frames equal to approximately $150 available every twenty-four (24) months. Contact lens may be selected instead of glasses with a $150 allowance.
